Primary Health Properties PLC (PHP) is a UK Real Estate Investment Trust, and leading investor in modern primary care premises. They invest in flexible, modern properties for local primary healthcare in the UK and Ireland.
PHP is providing their Community Impact Program to promote grants to charities in the North-West focused on social prescribing and community wellbeing in the vicinity of their tenant occupied primary healthcare centres.
The NHS funds GP practices to employ social prescribing link workers. These link workers will receive patients, referred from the GP’s and other agencies. They will seek to understand the non-clinical needs of the patient and refer them to the most appropriate Community Groups and statutory services for their needs. Sometimes the community groups which the link workers refer patients, don’t exist or need funding themselves. PHP is setting aside certain funds to create, support or enhance the community services to which the social prescribing link workers of their practices would like to refer their patients to.
This grant programme is provided by Community Foundations for Lancashire and Merseyside, however the fund is open to groups based in Cheshire, Cumbria, Greater Manchester, Lancashire and Merseyside that meet the fund criteria below.

Please note that applications which do not evidence that they are working directly with a PHP Healthcare Centre to support PHP Healthcare Centre patients will not be considered.

The closing date to receive applications is Friday 14th October 2022 at 4pm.
Who can apply:
Registered charities, not for profit companies limited by guarantee, community groups with a constitution or set of rules in the group’s name.
Additional Information
Successful applicants will be required to submit interim monitoring about the progress of the project.

All completed applications are assessed by Community Foundation staff to confirm eligibility. You may be contacted for more information after applying and a decision making panel will make recommendations based on the fund’s criteria and the budget available.
You should expect to hear the outcome of your application in approximately 8 weeks from the closing date.
